Acknowledgement

I hereby authorize Photography by Tim Ellis to publish photographs and any related forms of electronic media captured of me, and my name, and likeness, for use in Photography by Tim Ellis's print, online and video-based marketing materials, as well as other publications.

Important Notice Regarding Copyright

It is illegal to copy or reproduce the Clients photographs elsewhere without Photography by Tim Ellis's permission. Violators of this Federal Law will be subject to civil and criminal penalties. Photographs on both Photography by Tim Ellis's website, Facebook page, blog, or Instagram including all potential proofs and purchased prints, are copyrighted. Reproduction of any image by any means, including copying, screen captures, reprinting and any other means of print reproduction are prohibited and are a violation of copyright law. It is illegal to post any of Photography by Tim Ellis work on the internet without the permission of the photographer. The only exception to this is if you have purchased the print release digital files provided by Photography by Tim Ellis.

In Agreement *

I understand that my likeness may be utilized by Photography by Tim Ellis for business objectives, including, but not limited to marketing, promotional, educational, instructional purposes, prints, calendars, books, magazines, playing cards, greeting cards, advertisement, submissions to journalists, website, social networking sites and other print and digital communications, without compensation to me

I waive any right to inspect or approve the finished product or material in which my likeness is utilized. There is no time limit on the validity of this release nor is there any geographic limitations on where my likeness is displayed

I understand that I may revoke this authorization at any time by notifying Photography by Tim Ellis in writing. The revocation will not affect any actions taken before the receipt of this written notification. They will be kept as long as they are relevant and after that time destroyed or archived.

I further represent and declare that this release contains the entire agreement between myself and Photography by Tim Ellis in relation to any claims that may arise from the usage of my likeness; and that the terms of this release are contractual and are not a mere recital.

I am releasing and discharging Photography by Tim Ellis together along with all of their principals, shareholders, officers, employees, agents, successors, and assignees from any and all liability arising out of or in connection with m appearance in the images or the subsequent reproduction and distribution of the images in which I appear in any medium.
